How the S/FMC program works with OneCare

The Self and Family Managed Care (S/FMC) program allows eligible Manitoba residents to choose and manage their own home care provider — including One Call Care at Home. Here's how the process works:

1. 1Live in your own home (private residence, apartment, or assisted living)
2. 2Apply for the program — call WRHA Intake Line: (204) 788-8330
3. 3OneCare can help complete your application form at no extra cost
4. 4Eligibility is assessed — a coordinator determines approved hours
5. 5Open a separate bank account for the government funding
6. 6Receive funding — keep all invoices (OneCare provides documentation)
7. 7Health region services (nursing, therapy, medical supplies) continue from your RHA
